Check the tube map here for stations with disabled access,
http://www.tfl.gov.uk/assets/downloads/standard-tube-map.gif . Most of
Central London is a bust but the newer sections i.e. the DLR, Jubilee
Line from Westminster towards Stratford and the like have lifts now.
Searching www.tfl.gov.uk pointed me to http://www.directenquiries.com/
which has a bunch of info on access at stations.

But the buses in London during the day are much much better than they
were since the congestion charge was introduced and the big bendy
buses have masses of space for buggies in them. So check the journey
planner at tfl.gov.uk, you can even tell it you don't want to use
stairs.
