[BNM] General purpose form validation script?

It doesn't support as many form elements as I'd like. Adding in initial  
values from a database isn't natively supported. It's a great starting  
point, but it's not finished code yet. I rewrote and expanded it a few  
years ago and it's fairly easy to work with.
